As a Project Manager myself, I totally agree with you.
Actually speaking we realize and understand all these points and there impacts very well, but practically in real project scenarios, they are very difficult to follow and that’s the reason most of the projects go over schedule.

I would also like to add few more points in this list:

1. All the project resources (internal or consultants) should be interviewed by the project manager as well as technical SME’s. In case working with vendors for resourcing, project manager should not hesitate to ask for multiple options before finalizing the best suited resource for the project. Also the expectations of team members should be set correctly in the beginning of the project.

2. Rewards & Appreciations
While you already covered this point under motivating the team, few regular rewards (monthly/quarterly) and appreciations helps the team. A small gift or appreciation email matters a lot.

Each project and situation is different and a positive attitude helps a lot a project manager to come out of any critical issue.

I agree with you, it is difficult to follow best practices in real life scenarios. But when practiced with discipline they can really turnaround the project. I have done it, achieved goals and that is why shared them here.
